• First and Last Name
• A copy of your driver's license or state ID
• A copy of your health insurance card
• Days and times you are available for ongoing sessions (please note: your scheduled time will be the same each week unless you need to reschedule). 

If this is an emergency, please go to your nearest emergency room or dial "988". 

We will be in touch during our normal business hours.
